Diabetes Intake-LPN

SUMMARY: SUMMARY: Receives and schedules all referrals to the Diabetes Self-Management Education and Support (DSMES) and the outpatient Medical Nutritional Therapy (MNT) departments. Obtains both initial and ongoing insurance authorizations for DSMES and MNT in coordination with the Diabetes program coordinator and Dietitian. Assists with patient calls and physician office calls. Assists the administrative and clinical staff in maintaining a patient/customer focus, supports the delivery of the highest quality care, shares a passion for patient and customer centered care, and assists in meeting or exceeding patient satisfaction and financial and operational targets.

EXPERIENCE: Medical office or Acute Care Hospital experience an asset. Insurance authorization experience preferred. SPECIAL SKILLS: Electronic Medical Documentation and Electronic Medical Record experience preferred.

OTHER: Excellent customer service skills; self-motivated, organized, and ability to work independently; computer skills.

LICENSE/CERT: Required: LPN (Licensed Practical Nurse), BLS (Basic Life Support) from AHA (American Heart Association)

EDUCATION: Required: High School or Equivalent
