Behavioral Health NavigatOR (PRN)

SUMMARY: The Behavioral Health Navigator works in a collaborative fashion with patients, families, caregivers, and the multidisciplinary team to ensure behavioral health patients receive comprehensive, timely, cost effective, quality health care. The Behavioral Health Navigator will work in coordination with the psychiatric physicians, nursing staff and case managers/social workers to assist with appropriate resources and facilitate appropriate discharge plans.

EXPERIENCE:Mental/Behavioral Health experience required. Minimum 2-3 years� experience in hospital Social Work or related healthcare field.

OTHER: Excellent communication, interpersonal, organizational and assessment skills. Knowledge of community resources and programs.

LICENSE/CERT: Required: LCSW (Licensed Clinical Social Worker)

EDUCATION: Required: Masters - Social Work (CSWE approved)
