HH Documentation Specialist

SUMMARY: Under the direction of the Supervisor of Home Health Documentation, conducts quality audit and documentation review activities as assigned. Assists with providing education to staff on documentation related issues. Facilitates case conferences as assigned.

EXPERIENCE: Minimum 3 years of home care experience.

SPECIAL SKILLS: Computer skills. Strong verbal, written, and interpersonal communication skills, and ability to educate. Strong case management knowledge and skills.

OTHER: 2 years OASIS experience required, current driver�s license and auto liability insurance.

LICENSE/CERT: Required: RN (Registered Nurse) State of Missouri, PT (Physical Therapist), OTR (Occupational Therapist), OASIS (Outcome and Assessment Information Set) within 1 year of hire

EDUCATION: Required: Bachelors - Nursing, Bachelors - Physical Therapy, Bachelors - Occupational Therapy
