Summary

Meritas Health, a subsidiary of North Kansas City Hospital, is currently seeking a Coordinator of Quality Outcomes to join our Corporate team!  This position works to improve physician network performance by monitoring and auditing quality data related to preventive and chronic care conditions, as well as age appropriate screenings and immunizations, to support the quality programs and initiatives for Meritas Health.

Qualifications:

  • Bachelor’s in Nursing required
  • Current Missouri Registered Nurse license
  • Knowledge of quality improvement principals with 3-5 years’ experience in health care and quality improvement initiatives
  • BLS through American Heart Association
  • Experience in clinical education preferred
  • Ability to plan, coordinate and organize multiple priorities
  • Strong analytical and organizational skills
  • Strong attention to detail
  • Self-motivated and able to work independently
  • Ability to maintain focus on long range goals
  • Exceptional written, telephonic, and oral communication skills
  • Proficiency using Microsoft Office programs, especially Excel, Word, Power Point
  • Excellent customer service skills

Job Duties:

  • Monitors performance indicators and analyzes data patterns/trends to assist medical practices in achieving quality improvement goals.
  • Learns and understands various quality programs impacting ambulatory practices such as patient centered medical home requirements, meaningful use, HEDIS measures, and various health plan/payer groups.
  • Utilizes available electronic tools, such as electronic medical records (EMR), population health databases, and health information exchanges to complete quality/performance improvement initiatives.
  • Researches, maintains and disseminates up-to-date reference materials and files related to health plan requirements.
  • Participates in the medical practices’ performance process improvement initiatives, including conducting patient chart reviews, special audits, report validation and documenting performance gains.
  • Collaborates with medical practices to ensure ongoing compliance and then identifies and assists in the development of educational opportunities for practice staff.
  • Analyzes outcomes of quality improvement activities, identifying barriers to success and develops and implements methods to mitigate those barriers.
  • Maintains strong working knowledge of all EMR workflows and facilitates the clinical new hire orientation and EMR training as needed.
