JOB SUMMARY:  The LPN Care Coordinator will report to the Practice Administrator and is responsible for direct patient care (90%) and care coordination (10%).  The percentage will flex depending on business need and staffing.  

RESPONSIBILITIES:

  • Supports Providers to maximize patient care
  • Talks directly to patients regarding medical concerns and advises on care as directed by provider
  • Provides phone triage
  • Approves or denies medication refill requests, communicates with pharmacies to resolve medication concerns, works with providers for approval on medications outside of protocols
  • May administer medications, perform diagnostic and therapeutic procedures
  • Partners with other nurses and physicians for care coordination for patients who require additional follow up or services
  • Patient Education
  • Communication with home health staff, nursing home/SNF/rehab staff regarding mutual patients

MINIMUM REQUIREMENTS:

  • Certified Medical Assistant, LPN or RN with ability to practice in MO required
  • High School Diploma or equivalent, Associate’s Degree in health care related field preferred
  • Knowledge of medical terminology, organizational skills, strong clerical and computer skills, excellent customer service skills
  • 1-3 years in a medical office setting
  • Knowledge of Cerner EMR is a plus
  • BLS through American Heart Association required
  • Ability to communicate effectively with patients and coworkers
  • Provides patients with excellent customer service
